For booking and payment authorization please click on the "Payment Authorization" link below.

When Filling Out the Authorization, Please Remember:

• Please select “payment” as the reason for filling out the form.

• Add my name (Kenley Sweat) to the agent drop down box so that the authorization is sent to me in a timely fashion.

• Make sure to include your name, address, contact information and payment details.

• In the “Reservation Information” section: please indicate that this is a new reservation and add the Full Legal Names and Date of Birth for each Guest.

• Please indicate if you are paying the deposit, or if you are paying the full amount due.

• If traveling Internationally or by Cruise please add all travelers passport numbers and expiration dates to the "comments" box. Make sure that the names indicated on the form match your passports identically.

**Please double check accuracy of spelling and dates of birth. If traveling internationally this must match your Passport EXACTLY. For domestic travel, this must match your Driver's License. This information will be used to make your reservation. Errors may result in supplier penalties or inability to travel.

Once this has been completed I will book your vacation and send you all confirmation information and details!
